2023 Summer and Fall Physical Sign Up Instructions:

Sports physicals will be held on May 23rd at 2pm by the Gym entrance. Students will need to sign up by May 15th in order to receive a physical.  Because physicals are limited to the first 80 students that sign up; there may be an additional physical day added in June, if necessary. To sign up for a school physical all athletes must complete the following:

1. Attend a sport specific interest meeting.
2. Register for a sport before the May 15th deadline.
3. Turn in the first two pages of the Preparticipation Physical Exam (50 Health History Questions)
4. Turn in the Covid Form 1 
5. Turn in the Additional Covid-19 Clearance Form (If Applicable)

All paperwork turned in must be placed in Mrs. Mcknight's mailbox or given directly to the Athletic Secretary. The athlete should directly ask to be placed on the physical sign-up list when turning in paperwork. All students can verify if they are on the list and approved to get a school physical by contacting the Athletic Trainer. Students that sign up for a physical but their current physical does not expire before October 30th, will not be placed on the physical sign up list.

If not signing up for a school physical because you already have a valid and approved physical for the entire Fall season, you still must complete the following:

1. Attend a sport specific interest meeting.
2. Register for a sport before June 5th
3. Turn in the first two pages of the Preparticipation Physical Exam (50 Health History Questions) and the Covid Form 2.
4. Turn in the Covid Form 1 
5. Turn in the Additional Covid-19 Clearance (If Applicable)
6. Turn in any supporting forms that are applicable to you (Asthma Action Plan, Asthma Denial Form, Diabetic Action Plan, Orthopedic Clearance, ec.)

* All paperwork must be turned in prior to June 5, 2023 if wishing to participate in Summer practices in June or July.
